Chronic Disease Self-Mangement Program




Better Choices, Better Health

Are you or a loved one living with an ongoing health condition such as diabetes, arthritis, high blood pressure, heart disease, chronic pain, or anxiety? If so, take charge of your life with a Chronic Disease Self-Management Program workshop offered through a partnership of the Black Hawk County Health Department and the Hawkeye Valley Area Agency on Aging. The Better Choices, Better Health program features a series of 6 weekly workshops, 2 ½ hours in length. Topics include managing symptoms, medications, how to relax, problem solving, safe & easy exercises, tips for eating well, and setting goals. A “materials” fee covers the program book and relaxation cd.
