Are you or a loved one living with an ongoing health condition such as diabetes, arthritis, high blood pressure, heart disease, chronic pain, or anxiety? If so, take charge of your life with a Chronic Disease Self-Management Program workshop offered through a partnership of the Black Hawk County Health Department and the Hawkeye Valley Area Agency on Aging. The Better Choices, Better Health program features a series of 6 weekly workshops, 2 ½ hours in length. Topics include managing symptoms, medications, how to relax, problem solving, safe & easy exercises, tips for eating well, and setting goals. A “materials” fee covers the program book and relaxation cd. For more information or to register for an upcoming class, please contact our partner agency, Hawkeye Valley Area Agency on Aging at 319-291-2413.
An online internet-based workshop is also available. Contact BetterChoices@selfmanage.org for more information on the internet version.
