Important Notice Regarding Your Life Insurance Benefit

Did you know that the life insurance benefit provided to you by the County has a provision called the "Waiver of Life Premium Benefit?"

This means that if you become totally disabled prior to age 60, Madison National Life Insurance Company may waive the insurance premium for you so that you may continue to have your life insurance benefit at no cost to you.

To receive this benefit, you must submit a completed application/claim form to Madison National Life Insurance Company prior to terminating your employment with Black Hawk County. Once your application is received, Madison National Life will investigate your disability claim and if they find you to be totally disabled, they will notify you of your benefit coverage. If they find that you are not totally disabled, you may be eligible for conversion coverage, which allows you to convert the coverage to an individual policy.
